BackgroundEarly in the COVID-19 pandemic, the urgent need to discover effective therapies for COVID-19 prompted questions about the ethical problem of randomization along with its widely accepted solution: equipoise. In this scoping review, uses of equipoise in discussions of randomized controlled trials of COVID-19 therapies are evaluated to answer three questions. First, how has equipoise been applied to COVID-19 research? Second, has equipoise been employed accurately? And third, do concerns about equipoise pose a barrier to the ethical conduct of COVID-19 (...) RCTs?MethodsGoogle Scholar and Pubmed were searched for articles containing substantial discussion about equipoise and COVID-19 RCTs. 347 article titles were screened, 91 full text articles were assessed, and 48 articles were included. Uses of equipoise were analyzed and abstracted into seven categories.Results and discussionApproximately two-thirds of articles used equipoise in a way that is consistent with the concept. They invoked equipoise to support RCTs of specific therapies, RCTs in general, and the early termination of RCTs after achieving the primary outcome. Approximately one-third of articles used equipoise in a manner that is inconsistent with the concept. These articles argued that physician preference, widespread use of unproven therapies, patient preference, or expectation of therapeutic benefit may undermine equipoise and render RCTs unethical. In each case, the purported ethical problem can be resolved by correcting the use of equipoise.ConclusionsOur findings highlight the continued relevance of equipoise as it supports the conduct of well-conceived RCTs and provides moral guidance to physicians and researchers as they search for effective therapies for COVID-19. The Mississippi River flows through American history and culture as a mythic waterway brimming with tragedy and hope, and awash in passionate ambitions and harsh realities. In 1953, a young Charles Dee Sharp traveled twice down the Mississippi to make a documentary film of it, taking black-and-white photographs of the river, its communities, and its people. While Sharp’s documentary never came to fruition, the striking images he captured survived as moving and evocative historical testaments to a lost era, now (...) collected in his new book The Mississippi in 1953. These images create a vivid portrait of America’s heartland a half century ago, and they are enriched with excerpts from Sharp’s original trip journal, intriguing anecdotes from the people he encountered along his journey, and an engaging environmental history of the river by historian John O. Anfinson. The Mississippi in 1953 offers an original and poignant look at the living artery of the American landscape and how it molded the United States into the nation it is today. (shrink)

This paper traces the history of the International Society for Universal Dialogue by reflecting on the tension between universalism and pluralism and the underlying dialectics of identity and difference. This paper argues that this tension is the source of creativity and that dialogue, by its refusal to privilege one over the other, keeps this tension alive as it seeks ever better formulations and understandings of goodness, justice, and truth. This paper argues that philosophers are duty bound to honor their ideals (...) and values through the sort of reflection and dialogue that features critique, clarification, and renewal of those ideals and values. Procopius employs the motif of “grieving in silence” to describe the deliberations preceding Justinian’s invasion of Vandal North Africa in 533 (Wars 3.10.7-8) and his vendetta against the urban prefect of Constantinople in 523 (HA 9.41). The particularity of Procopius’ language in these passages makes their collocation especially pronounced. The distance between the Wars and the Secret History, which represents itself breaking the silence between what the Wars can state publicly and the unvarnished truth (HA 1.1-10), may be measured by (...) two “wise advisers” who speak when others are silent: the quaestor Proclus, warmly remembered for his probity, and the praetorian prefect John the Cappadocian, a figure universally reviled. Discontinuities between the presentation of John in the Wars and the merits of the policies he endorses problematize readers’ impressions of not only John but also the relationship between the Wars and the historical reality the work claims to represent. Pluta dismisses out of hand my suggestion that her mistakes are the result of the particular lens through which she and much of the political science community view the American presidency. Yet this suggestion has the merit of explaining why she contends that piling up nineteenth-century instances of presidential public “speech” undermines Jeffrey Tulis’s contention that the nature of presidential speech changed decisively at the (...) beginning of the twentieth century, such that rhetorical, often-demagogic appeals over the heads of Congress displaced public speech that affirmed the values of republican, constitutional government. The aims of this book are: to explain the notion of phenomenal consciousness in a non-metaphorical way that minimizes controversial assumptions; to characterize the relationship between the phenomenal character and intentionality of visual experience, visual imagery and non-imagistic thought; and to clarify the way in which conscious experience is intrinsically valuable to us. It argues for the legitimacy of a first-person approach to these issues--one which relies on a distinctively first-person warrant for judgments about one's own experience. Thought experiments are (...) employed in which one is asked to conceive of having various forms of blindsight, so as to make consciousness intellectually conspicuous by its absence in such hypothetical scenarios. It is argued that theories of mind that would commit us to denying either the conceptual or the metaphysical possibility of these scenarios neglect the occurrence of consciousness in this phenomenal sense.
